Are Health Advocacy-type benefits to be in 5500s?

The health advocacy type benefits seem to be in a grey area to include a Schedule A or not. This benefit conducts the following: helps locate providers, addresses errors on bills/claims, answers questions on coverage, and asissts with denials and appeals. Would you include a Schedule A? (Plan Documents and SPD not available for referencing).

Did you receive a Schedule A from the company providing the service?  If not, did they provide you with all the information necessary to complete the Schedule?  It sounds similar to an ASO contract which would not be included.

And if not Schedule A, does the plan's administrator have enough information to complete Schedule C regarding the plan's payments for the health-advocacy services?  Or did the employer pay without using a plan's assets?

The broker is asking if a Schedule A should be collected. The plan is fully-insured.

I would request to be safe.  If they deny providing or enough information to prepare, then you can always report that denial.  My hunch is you don’t have to report in Schedule A, but that is only because similar ASO services are not reported
