  • History and Info
    Pexeva (Paroxetine) is the first FDA approved non-hormonal treatment for hot flashes associated with menopause.   Pexeva contains the active ingredient in Paxil and while the clinical trial showed that Pexeva ‘reduced hot flashes compared to placebo,’ ‘the mechanism by which Pexeva reduces hot flashes is not known.’


    The FDA’s Reproductive Health Drugs Advisory Committee voted 10 to 4 that the overall risk-benefit profile of the drug did not support approval – that the benefits over placebo were minimal and that the severe side effects associated with Selective Serotonin Reuptake Inhibitors (SSRIs), such as suicidal thoughts and osteoporosis, outweigh the benefit.   

    The FDA’s decision to approve Pexeva ran counter to their panel’s recommendations (which is highly unusual) and the agency gave no explanation as to their decision to approve Pexeva.

    Pexeva contains 7.5mg of Paroxetine and at week 12 in one study, there was a small reduction from baseline of 5.9 moderate-to-severe hot flushes per day as compared with a reduction of 5 per day with placebo.

    Paroxetine is one of the most potent of the SSRIs and has shown a high incidence of dependence and was ranked by the World Health Organization in 2001 as the most difficult antidepressant to withdraw from.

    Paroxetine has been associated with a high incidence and severity of withdrawal symptoms known as a discontinuation syndrome and it is therefore recommended to taper gradually to minimize the symptoms.  Many find that even with a slow reduction the withdrawals are too severe.
